Job Title: Social Worker 

Organization: The Transition Academy 

Location: Kansas City, Missouri 

Reports to:  Social Services Manager 

Job Summary: As a School-Based Social Worker you will work with urban high school Juniors and Seniors  with cognitive and developmental disabilities in the Kansas City metro area. In this role, you will be instrumental in helping students obtain employment and develop essential transition skills. Will work closely with students, families, and educators to identify individual needs and create personalized plans that support successful transitions from school to employment or further education.

In addition to career and transition planning, you will incorporate social-emotional and mental health into your services. By addressing students’ social, emotional and mental health needs, you will help them build resilience, develop healthy relationships, and enhance their self-awareness and self-management skills. This holistic approach ensures that students are not only prepared for the workforce but also equipped to navigate life’s challenges with confidence.

Essential functions of the job may include but are not limited to the following:

* Provides direct and indirect School Social Work services to students with IEPs. Such as  short-term individual or group counseling to assist students for the workforce or college. 

* Provides information to students and parents/guardians about services available within the community. Makes referrals to community providers, as appropriate.

* Maintains an accurate record of student, parent/guardian, and other contacts.

*Ability to travel between and among school sites throughout the city as necessary.

*Ability to be flexible and prioritize activities.

*Ability to communicate effectively with a diverse group of clients and community members.

*Participate in team meetings, case conferences, and training sessions.

*Perform other duties as assigned by the supervisor.

EDUCATION OR EXPERIENCE:

Preferred: Education/Certification: Master’s degree in Social Work, with an active license.

Required: Bachelor’s degree being in behavioral health or human services preferred (psychology, rehabilitation, social work, marriage and family therapy, counseling).

Special Knowledge/Skills: Human development, mental health, behavior interventions, relationship building, strong communication skills both orally and written.

Proven experience working with youth with disabilities, particularly within communities of color.

Salary:

$50,000-$55,000 per year
