The Transition Academy is a post-secondary navigation tool that students and families can use to explore college and career opportunities. This model is innovative because it transfers the knowledge and power from social services, schools and personal networks to where it rightfully belongs – in the hands of families.

VISTA will work with schools and government benefit coordinators in setting up processes and systems to ensure students receive the benefits they are eligible for (i.e. Medicaid [health care and job waivers], transportation, housing, and food stamps. They will develop partnerships with the schools’ social workers and process coordinators to ensure students and families are aware of these programs.
